This form came from Ann Brandt by way of Mary Leskovsky.  It offers a variation on assessment that isn't in the text but one that would still work well for the Basic Option Plus.  Although it is focused for a literature course, revisions could easily be done to make it fit any course.]

Your role in this activity is to provide me with feedback regarding the information you, as a student, found the most or least interesting or useful.  This is not a graded assignment; however, your honest reflection on the following statements will help me in planning future classes.  Your name isn't required on this assessment.

The following is designed to help focus on the processes that may help or hinder understanding of the literature studied this semester.  Please be honest in your answers--they are in no way related to your grade.
