This web site provides a shorcut for setting up a web page form that will send user input to one's e-mail (GCC student account or MEMO only), as described at http://www.gc.maricopa.edu/webdev/digest/faq7.htm. Upon completing the steps below, you will get a basic web-ready form depending on the content you provide. You may then paste the form into your own web page design, via Netscape Composer or HTML. At that point you may choose to further customize the appearance of the form -- add line numbers, choose to group certain fields on the same line, etc. -- beyond the options given here. The form will be functional on pages published on GCC's GCWeb, Glory, Gecko, and NS web servers only.

What to do first:

• In a separate application such as Wordpad or Simpletext, type up the general content of your form. For example, if the form consists of a series of questions to which the user must respond, type the questions in the order you want them to appear. Then figure out what format each answer should take: multiple choice, freeform text, etc. (The standard form input types are listed below.) Having this source of information prepared in advance will make it much easier to proceed here.
• Determine, in advance, the complete URL of the form -- i.e. your web site address, ending with the file name of the web page that will contain the form (whether it already exists or not). One approach to this is to post a web page with an empty spot where you plan to later insert the form, and plan to use the address of that page.
• Create a "thank you" page and publish it separately on your web site. This page will appear for the user after the final submission of the form. At minimum, it lets them know the submission process worked, that their information has been delivered. Such a page should also include links back to your web site and any additional directions the submitter should follow as needed. You will need the URL of this page while constructing the form.
• What are the e-mail addresses of all who will receive copies of form submissions via e-mail? What subject line is best (especially for filtering submissions into a custom mail folder)? This information is needed while constructing the form.
