Extension Request CIS133AA, CIS133BA and CIS133CA

The following conditions MUST be meet in order for you to have your end date extended.  You are permitted ONLY ONE extension of 8 weeks (school weeks when classes are in session) past your present end date.
  1. Completed the minimum amount of work as required for your specific course.
    1. CIS133AA - Project 1, Project 2, Quiz 1 and Quiz 2
    2. CIS133BA - Project 1, Project 2, Quiz 1 and Quiz 2
    3. CIS133CA - Project 1 and Quiz 1
  2. Request the extension by completely filling in and submitting THIS FORM.
Do not call, email or use the green forms in the High Tech Centers to request extensions.

You should receive an email reply in several days letting you know the status of your extension request.
