This will be a hand drawn pencil and paper Entity Relationship diagram.  You will NOT do this diagram using a computer.  Your diagram should:

PART TWO:
You will write relationship sentences to describe every relationship.  These must follow the format discussed in class. This part may be done on a computer using Word.

Staple your work to the Project One cover sheet.

OUR COMPANY

Our company is a fast growing full service animal care facility located in north Phoenix.  We two types of services: medical care and boarding of pets.  We provide the best in medical care with our two full-time veterinarians (Roger Neptune and Virginia Henson).  We have four veterinarians assistants - Sarah Jones, Karl Fission, Lena Capiano and Jose Juarez.  We have a trained and caring staff of Nancy Neptune (Roger's wife), Diane Fox, Albert Williams who manage all phases of a pets stay in our facility.  Also, Lora Brooks, Phil Henson (Virginia's husband), Karen Zak and Roberta Walker take care of the administrative duties such as receptionist, billing, appointment scheduling and inventory control.

We welcome all common pets - dogs, cats, hamsters, rabbits, guinea pigs and small birds.  For boarding we only handle dogs and cats at present.  However, we do not handle for either medical or boarding "exotic" pets,  such as snakes, turtles or lizards.

Our customers are very important to us.  Therefore, we keep information on any owner that brings in a pet.  We want to know their name, address, and phone number(s).  Each customer is identified by using their last initial, first initial and the last four digits of the phone number.  Someone like Linden Dale Peterson whose phone number is 555-3456, would be identified by LP3456.  This system is one that we have used for years and never has caused any problems.

For every pet, we must know the pet's official name.  Many people have registered their pet with an organization like for a dog the American Kennel Society.  If they do not have a registered name, than the pet's official name will be the pet's common name plus the owner's last name  We want to know what name is used on a day to day basis for the pet.  For example, Mr. Peterson's cat has an official name of "Oscar Dale the Great".   But Mr. Peterson calls his cat "Sleepy O".

Other important things that we must to know about the pet include the pet's type - whether its is a dog or cat  etc.   Also, we must know its breed (for a dog it would be things like collie, french poodle, etc.) and gender.  We would like to know the pet's age, weight and color.

When people bring their pets in for medical treatment we must have accurate and up-to-date medical history (ex. allergies, shots, operations and procedures).  We keep track of the date of each visit and what treatment was given for what specific symptoms.  We want to know how the pet behaves during its treatment - e.g. calm, nervous, angry.

If a pet will be staying in our facility, then we musy know when it will start its stay and for how many days it will be with us.  A very important item that we must know is the size of the pet as it related to what size pen it should stay in.  We do not want a customer to come in with their saint bernard and have all of our larger pens filled with small dogs and cats and have plenty of small pens open.

Also, information that we would like to know is the pet's diet and exercise needs.  Additionally does the pet have any special items (e.g. toy or blanket) or any other special concerns (e.g. must be brushed every day).

We can take reservations for both medical appointments and for stays in our facility.  We accept walk-ins for both medical care and stays in the kennel on an "as-available" basis.  We encourage the advanced registrations by giving those people a discount on their pet's stay.  In order to identify a specific reservation we use a combination of the customer's information, pet information and the date and time of the visit or date that the stay starts and its duration.

We ask for payment upon the completion of a service (e.g. medical appointment or stay in the kennel).  However, we will keep a monthly bill for people who have major expenses or who ask that we do so.  We want our customers to be happy and more importantly we want their pets to be happy and healthy.

We have plans to expand into several new area in the near future.  However, for the present time this is all that we are going to be keeping track of by using the computer.
