Online Clinical Requirements
All GCC EMT Students:
1. All clinical prerequisites must be met before you are eligible to sign up for clinicals. Communicate directly with your instructor to determine if you have completed all necessary prerequisites.
2. When all prerequisites have been met, your instructor will provide you with a signed clinical badge authorization form. You must bring this form to the Enrollment Center at the main campus to receive your clinical badge.
3. Bring the clinical evaluation form and patient contact record with you to your clinical. The completed forms must be submitted to your instructor at the next scheduled class session.
If you are going to Arrowhead Hospital:
1. 18701 N. 67th Avenue in Glendale, Arizona, 85308
2. You must view the Arrowhead Corporate Compliance Video.
3. You must read the Arrowhead Handbook. Complete the following tests and provide to your instructor before attending Arrowhead Hospital:
a. Infection Control Posttest
b. Safety Posttest
c. Abrazo Compliance Test
d. Abrazo Advanced HIPAA Study Sheet
4. Visitor parking is available on the west side of the hospital (see map).
5. During your clinical, Arrowhead Hospital offers a 20% cafeteria discount to students who have their clinical ID badge.
If you are going to JCL North Mountain:
1. 250 East Dunlap Avenue Phoenix, AZ 85020
2. You must read the JCL Student Guide and complete the following:
a. Evaluation of Student Experience (provide to instructor after clinical)
b. Student Responsibility Statement
3. Parking is available on the south side of the Emergency Department or the parking garage on the north east corner of 3rd Street and Hatcher (see map).
If you are going to JCL Deer Valley:
1. 19829 North 27th Avenue Phoenix, AZ 85027
2. You must read the JCL Student Guide and complete the following:
a. Evaluation of Student Experience (provide to instructor after clinical)
b. Student Responsibility Statement
c. Mandatory Annual Safety and Health Test
3. Parking is available on the north side of the hospital, near the Wee Care building (see map below). Do NOT park in front of the parking garage.
