Fire Science Technology Registration and Application Forms

The following documents are in PDF format, requiring Adobe Acrobat Reader to open. If Adobe Acrobat Reader is not already installed on your computer, you may download it for free from the Adobe web site.

• CPAT Applications for 2012  Register in the PSS faculty office at the GCC campus. A card with your specific time will be mailed to you 1 week prior to the test.

• CPAT Applications for 2013 Register in the PSS faculty office at the GCC campus. A card with your specific time will be mailed to you 1 week prior to the test.

To register for the CPAT, you must fill out paperwork in the PSS faculty building on the North side of the GCC campus. For questions regarding the CPAT or office hours, please contact  Cindy Rosales @ 623-845-3161. All orientations, practice tests and CPAT tests will be held at the Glendale Public Safety Training Center located at 11550 W. Glendale Avenue. No phone, mail or fax registration will be permitted. You must register in person to sign the waivers.

• Fire Academy Applications: YOU MUST HAVE YOUR EMT 104, CPAT within 6 months, and physical within 3 months BEFORE TURNING IN YOUR APPLICATION. Applications must be brought in person to Edward Muller at the Glendale Regional Public Safety Training Center located at 11550 West Glendale Avenue.
