Fire Science Technology Registration and Application Forms

The following documents are in PDF format, requiring Adobe Acrobat Reader to open. If Adobe Acrobat Reader is not already installed on your computer, you may download it for free from the Adobe web site.

• CPAT Applications for 2009-2010  Register at the 04 faculty office at the GCC campus. A card with your specific time will be mailed to you 1 week prior to the test.

To register FOR the CPAT, you must fill out paperwork in the 04 faculty building on the North side of the GCC campus. For questions regarding the CPAT or office hours, please contact  Cindy Rosales @ 623-845-3161.

All orientations, practice tests and CPAT test will be held at the new Public Safety Training Center located at 11550 West Glendale Avenue.

No phone, mail or fax registration will be permitted. You must register in person to sign the waivers.

• Fire Academy Application (201 KB)  YOU MUST HAVE YOUR EMT 104 and a current CPAT BEFORE TURNING IN YOUR APPLICATION. The Fall 09 Fire Academy start date will be August 22nd, 2009. Applications must be brought to Sommer Pahl at the Glendale Training Center located at 11550 West Glendale Avenue. I will begin taking applications in July.
