Furniture Repair Form

Campus Requesters: Use this form to report and request a furniture repair. This form will be used as a basic tool to gather information. Your submission will be forwarded to the Procurement team and you will be contacted for an evaluation and scheduling. Please be advised that not all furniture is repairable or under warranty. You will be notified if the item is not salvageable.

Please provide a specific location of the broken item, this is especially helpful in an area where there is a significant amount of furniture. Please also leave a note on the broken item to identify it.

(i.e. chair height will not adjust, missing casters, etc.) Additionally, please specify if you need a replacement item while your item is being repaired.
