Surplus Request Form

Campus Requesters: Use this form to request pick-up of broken or unwanted furniture. This form will be used as a basic tool to gather information. Your submission will be forwarded to the Procurement team and you will be contacted for scheduling. If the furniture is tagged, please have a Property/Transfer Disposal Form ready at the time of pick-up. If the furniture is in good condition, the Procurement team will try to place it on campus prior to sending to surplus. We maintain a stand-by list of individuals looking for specific items. Once a surplus item has been picked up from your area, it is non-returnable.
