Bookstore Charge
Request Form
Bookstore Charge Requests are used to make purchases from the
bookstore.
They may be
obtained from the
Cashier's office or the Student Leadership Center.
They
may not be used for clothing.
When using a Bookstore
charge, a 25% discount is given off merchandise.
Be sure to get all required signatures and your
account code before going to the bookstore.
Instructions:
1. Obtain Bookstore Charge
Request from Cashiers or the Student Leadership Center.
2. Fill out top portion of
form including your Club/Organization's
CFS financial budget code number.
3. Obtain signatures from your
Faculty Advisor and the Dean of Student
Life and the College Business Office.
4. Submit form to the
Bookstore.
5. Bring your purchases to the
Bookstore cashier and present the Charge
Request.
6. Allow bookstore staff to
fill in the description and price.
7. Sign in the area designated
by the staff.
8. Turn in your copy to your
club treasurer.
