Information Form
(available in the Student Leadership Center)
Donor Information Forms are used anytime donations are received by your
club or student organization.
The form is a triplicate copy
form. The top (white) copy should go
to whomever made the donation at the
time it is received, the
middle (yellow) copy is to be retained by
your treasurer and saved for three
years with your group's
records, and the bottom (pink) copy is to be
sent with a thank you card to
whomever made the donation
following your event/activity.
