Elliot Cherner of Information Technologies Services at the District Office presents a web-based tool for quickly and easily creating a form for an on-line survey or quiz, with the resulting answers and statistics accessible to the form's author via a web interface.

Although the program is in its early design stage, attendees with no experience in web publishing can create a live on-line quiz or survey in minutes using the easy controls.
